We support every stage of the Continuing Healthcare and Decision Support Tool process, helping to identify eligible health needs, prepare clear evidence, liaise with professionals and Integrated Care Boards, and question or challenge care and funding decisions where appropriate.
Our social work and case management input also supports needs assessments, rehabilitation options, care planning, funding routes and related benefits.

- A free telephone consultation to identify whether Liberty can help, with free signposting to statutory support where appropriate.
- Care needs assessments across Leicestershire and the surrounding areas, including independent assessments of health, social care and home safety needs.
- Support with NHS Continuing Healthcare, local authority funding, assistive technology, occupational therapy referrals and care planning to help people remain safe at home.
- Advocacy to help ensure care is proportionate and supports the person in the best possible way. Where problems are identified, Liberty can support a review or appeal of the care or funding decision.
- Independent evidence to support Lasting Power of Attorney, deputyship and Court of Protection decision-making, including where a person’s capacity to manage their finances needs to be considered.
