Complex Care Case Management Across The East Midlands

We coordinate care and rehabilitation for people living with complex care needs, including acquired brain injuries, spinal injuries, physical disabilities, neurological conditions, complex behavioural needs, capacity concerns, safeguarding risks and high-level care requirements.

  • Comprehensive immediate needs assessments that identify risks, priorities, rehabilitation goals and the support required to promote safety, independence and quality of life
  • Person-centred, outcome-focused care and rehabilitation plans developed around the person’s needs, wishes, strengths and long-term aspirations
  • Coordinated input from therapists, care providers, statutory services, community resources and family networks to ensure support is joined up, proportionate and responsive
  • Care package design, implementation, monitoring and regular review to ensure support remains safe, effective, value for money and aligned with changing needs
  • Proactive safeguarding, risk assessment and contingency planning, including crisis prevention, escalation pathways and least restrictive approaches
  • Clear, evidence-based progress reporting for solicitors, deputies, families and professionals to support informed decision-making and accountability

Rights-based, person-centred practice with legal and local care knowledge

Our work starts with the person: what matters to them, what they want life to look like and what is preventing this from happening.

Liberty HSC applies a robust, rights-based approach grounded in the Care Act, Mental Capacity Act and Mental Health Act, promoting person-centred support, proportionate safeguarding, best interest decision-making, deprivation of liberty considerations, statutory assessment and the least restrictive care and support available.

We balance safety, choice and independence through a positive risk enablement approach, ensuring that risks are understood, managed and reviewed without unnecessarily limiting the person’s autonomy, dignity or quality of life.

We also help address barriers relating to care, communication, risk, capacity, mental health, family pressures, housing, transport, funding, statutory services and access to specialist therapy, drawing on local knowledge of social care, NHS services, mental health provision, community rehabilitation, private care providers and voluntary sector support.
